Integrated Care and Support Plan - High Level Process

This document describes the approach undertaken in creating a generic Integrated Care and Support Planning (ICSP) process, reflecting the broader business process re-design and including how CAF messaging would be used to support this. It has been developed through collaboration between three local authority/health partnerships, Connecting for Health, the Care Services Efficiency Delivery programme and Putting People First. The members of Barnsley Arena (a Service User- and Carer-Led Organisation) provided valuable input.
It should be noted that the model outlined is not a mandated process, but a generic high-level process, and it is acknowledged that it does not reflect the detailed variations found in local processes. Whilst the document reflects a generic 'customer journey', the focus is to describe how information can flow and be shared between health and social care systems.
