The Common Assessment Framework (CAF) for Adults aims to improve information sharing across organisations for multi-disciplinary assessment and care and support planning. This network is a hub for information sharing and collaboration for those interested in the CAF Demonstrator Programme and the Social Care Personal Demographics Service Early Adopter Programme.
